Help with Birth Cert?

My Gt. Grandfather Thomas Keatley was not with his family in the 1901 Census...I found him living with the widow Maggie McLellan(nee Fergus) and her 4 children(Her husband John McLellan died aged 33 in 1892)
After this, I discovered that in 1895 Maggie gave birth to an illegitimate boy, James Keatley McLellan, who died the same day.
Maggie died in 1903 aged 42.
Why else would an illegitimate birth have a surname as a middle name?

Can you help me decipher what it says under Fathers details...the date 12th Sept 1892 is her husbands date of death...what does it say above it?

To me it looks like James-Keathly Fergus or McLellan (illegitimate) and under details of parents it says Maggie Fergus, widow of John McLellan, pattern maker who died 12 September 1892.

Middle name is likely to show that she knew who the father was of the baby...probably your Thomas.
