I can see giving his own father's name by mistake - I've had a couple like that where they must have been confused by the question, although mine didn't go back to correct the record. Probably weren't even aware of it.

As to the second, I would like to think it was probably a case of all parties thinking of the future of the child and didn't think of the consequences if found out.
